Appointment Reminder Volunteer - Hammersmith & Fulham

Role Description

When patients miss appointments, it delays their care. Help our team understand why a patient might not be able to make an appointment and support them to find a solution so they can come. You could help them arrange a translator, change the date of their slot or advise on public transport.

You’ll be making a massive difference to a patients’ experience and help us reach more people who need our help.

Location: St Charles Centre for Health and Care 

Availability: 4 hours per week, between Monday-Friday 9am-5pm

 

Help the Nursing Team: 

  • Help the team reach patients with upcoming appointments
  • Support vulnerable patients to clearly understand where and when they have appointments, and work with them to find how they can best attend
  • Ensure that patients who are at risk of poor health outcomes get to see clinicians and access the care they need

  • Some previous NHS experience

  • Minimum 6 months commitment
  • Enhanced DBS Required
