Enhanced Support (Dementia) (18+ Only)

We have various opportunities to support patients with a dementia diagnosis in our ward areas. Within this role you can also support our patients who are living with dementia by reading to them or playing games. You will promote individualised and person-centred care, promoting and referring to the REACH OUT TO ME document. Also ensuring that families/carers are provided with a REACH OUT TO ME if the patient does not have one in place. Check that patients with dementia have a Blue Butterfly displayed above their bed. Check that patients with dementia have the correct cutlery, crockery and water jugs. Support the Admiral Nurse with any new dementia related initiatives. Support patients/carers to complete the Friends and Family Test and any other dementia related survey to help service improvement.
