Person Centred Care is an approach to the planning, delivery, and evaluation of health care that is based on partnerships between health care providers, patients, and families.
 
What is a Patient and Family Advisory Council (PFAC)?
Patient and Family Advisory Councils (PFACs) are regular meetings with patient partners (community members with a health care experience), executive health care leaders, and health care providers to provide strategic advice on advancing a person centred approach to care and services. The work of the PFAC is supported by Fraser Health values of respect, caring and trust, Our Shared Commitments, and We Together.
