Client Engagement Facilitator

  • With client contact sheet provided, utilize client data base to access client demographic information.
  • Educate and inform clients about ASDR services and supports
  • Through the process of completing a follow up phone call, re connect clients with our services and supports who previously self-identified the need for follow up support from the Alzheimer Society
  • Connect  clients with Family Support Staff who identify the need for counselling support
  • Register clients who wish to participate in educational workshops
  • Connect clients with Recreation Coordinator who wish to participate in social recreation programs
  • Complete data entry in client data base following client phone call
