PROGRAM: CARES Social Prescribing Program 

LOCATION(S): Various – Burnaby 

ABOUT THE PROGRAM:

The MOSAIC CARES program empowers seniors to enhance their health and well-being by connecting them to community resources that support their social, physical, and emotional needs. Through this program, seniors gain access to valuable non-medical services that complement their clinical care, helping them build meaningful connections, overcome barriers and enhance their overall quality of life. 

This program was developed to empower older adults to connect with social and community supports as a mean to enhance health and wellbeing and enable seniors to remain independent, connected, and active. 

PURPOSE:

The purpose of the Opportunity is to aid vulnerable seniors in the Burnaby area through phone and in-person supports by connecting seniors with volunteers and staff. Volunteers will help bridge the gap between seniors and the staff team, by supporting with information and referrals, friendly phone calls and friendly home visits, walks, emotional support and more. 

RESPONSIBILITIES:

  • Schedule 1-1 re-occurring meetings (phone or in-person) with the seniors
  • Friendly phone check-ins with seniors (30-45 minutes per week)
  • Option to provide in-person visits with seniors in public spaces (30-45 minutes per week)
  • Providing interpretation (if available) to connect and relay information with program staff and community programs (30-60 minutes bi-weekly or monthly)
  • Conduct frequent check-ins with CARES staff team regarding interaction with seniors and relay any questions or concerns
  • Responsibilities may vary depending on the type of event, typically this will be noted in the email callouts sent to volunteers

THIS ROLE WOULD BE SUITABLE FOR SOMEONE WHO:

  • Enjoys one-on-one interactions and conversations
  • Is available to volunteer (on a flexible schedule) during the weekdays from 9:00am to 5:00pm 

TIME COMMITMENT:

  • Commit to 1-1 re-occurring meetings with the Senior per week (Phone or In-person) - approximately 30-45 minutes per week
  • Available to provide interpretation (as required) with staff approximately 30-60 minutes bi-weekly or monthly
  • This role requires a minimum 3-month commitment
  • Although this role is flexible, we do require volunteers to commit on a weekly basis. Schedules may be determined by volunteers but must be confirmed by staff before committing to the schedule. 

REQUIREMENTS:

  • All applicants must go through our MOSAIC volunteer screening and onboarding process 
    • Screening process includes Volunteer Interview, Reference Checks, Criminal Record Check Clearance Letter, Agree to MOSAIC’s Confidentiality Policy and Volunteer Agreement
  • Complete mandatory CORE Orientation & Trainings and role-specific trainings which include but not limited to:
  • Completion of CARES Social Prescribing Orientation
  • Reviewing Emotional Supports, Motivational Interview, and Self Care Contents
  • Completion of working with seniors training
  • Completion of recognizing elders’ abuse
  • Volunteers are required to find their own mode of transportation to volunteer sites
