Family Connections - Assistant Family Care Coordinator
PURPOSE
To promote and assist family with obtaining what their child and family as a whole needs to be successfully in the home and in the community.
QUALIFICATIONS
Applicant must possess good verbal and written communication skills. Organization skills are helpful in this position as well as creativity and vision. Some computer skills are needed, particularity date entry and internet navigation.
DUTIES AND RESPONSIBILITIES
1. Build relationship with families
2. The FCC works with the family to develop a Child and Family Team.
3. The FCC will assist in determining what services and supports are needed by the family and coordinating those services.
4. The FCC will be responsible for facilitating the CFTM.
5. The FCC will create CFTM agenda.
6. The FCC is responsible for scheduling and preparation of all CFTM
7. The FCC is responsible for the SBA.
8. The FCC is responsible for own progress notes pertaining to services they render for family.
9. Attend all required staff development training and staff meeting
10. Work effectively within the team of service provider and professional.
11. All other responsibilities as required by supervisor
12. Participate in S- Team and IEP meetings.
13. Work in coordination with school staff other mental health staff and other concerned individuals.
14. Maintain on-going communication with supervisor to provide feedback in order to assess the need to modify the treatment plan and to address training needs.
15. Assist in maintaining client files.
ORIENTATION/TRAINING
Orientation will be conducted by the Volunteer Coordinator and take approximately 1.35 hours. Training will also be conducted by the Family Connection Supervisor and include topics such as alignment and engagement with families, community partnering, Child Family Team Meetings, Operation of the database, documentation expectations, Wraparound philosophy and System of Care.
TIME AND PLACE
This position will mostly be performed during office hours (8:00am to 5:00pm Monday through Friday) at the Chattanooga office. The exact hours of service will be flexible based on internship activities and number of hours available each week.
COMMITMENT
Estimated hours of services 25-35 hours per week (flexible schedule) with at least a one semester commitment.
SUPERVISION/EVALUATION
Supervision will be provided by Kristie Wilder and Dawn Carter. Performance evaluations are conducted semi-annually or as required by the internship.
