Modbury Hospital - GEM
This inpatient clinical unit focuses on the needs of the older person with complex medical, social and discharge planning needs. The unit’s focus is to promote, encourage and support the patients in regaining their independence while their medical issues are addressed. The GEM unit’s aim is to transition patient’s home but transition to longer term programs or residential care may be required. The target age group for patients admitted to the GEM unit are greater than 65 years or 55 years if Aboriginal or Torres Strait Islander.”
A Gem Ward Volunteer assists staff in providing regular groups for Ward patients and encouraging them to participate. This enhances the service provided by increasing individual patient support.
