Connect - Connect Resource Navigator
This position conducts research of local resources in order to continue to expand the Connect Resource Database, which supports the work that the Connect Coordinators do to provide supplemental case management and care coordination for Veterans, seniors, and families with children experiencing homelessness and housing insecurity. Individuals in this position provide direction to Connect Coordinators as needed when the care path for referred Hub guests is unclear.
Reports to: Connect Program Director
Schedule: Varies depending on availability and preference
Hours: 2-5 hours per week minimum required
