The Salvation Army Harbor Light Center
The Salvation Army Harbor Light Center provides a full continuum of services in the areas of addictions, transitional housing, spiritual development and supportive services designed to increase sufficiency. Since 1949, we have walked side by side with those suffering from addictions, leading them to full recovery. By God’s grace, our work continues today as we strive on becoming the leading provider of accessible drug, alcohol and gambling addiction rehabilitation services in Indiana. We are here to pave the way for our consumers to experience true, lasting freedom from the bondage of addiction.
Essential Duties
We are in need of help with answering of phones periodically, welcoming guests at the front desk, filing and shredding papers. We need someone need to be available to do this occasionally or consistently during the day for administrative support. Normal hours of operation are 8:00AM to 4:30PM Monday through Friday.
- Assist unit lead to formulates goals and objectives with the consumer by preparing a written treatment plan.
- Assist unit lead to prepare the treatment plan in accordance with the Biopsychosocial assessment.
- Assist unit lead to review goals every thirty days and on an ongoing basis and makes changes as needed for the consumer.
- Assist lead with documenting all progress toward goals and objectives on a progress or group note.
- Assist lead with documenting all consumer contacts in the case records, including progress notes, treatment plans, discharge summaries and discharge plans.
- Assist lead presenting the consumer’s case with the multi-disciplinary team and has the initial and updated treatment plans, and discharge summary signed by the supervisor.
- Assist lead with staffing any changes in consumers’ treatment goals and objective and obtains signatures.
- Participates in weekly staffing meetings to provide clinical input and observations as appropriate.
- Assist lead to counsel individual consumers in accordance with the goals developed on the treatment plan. (Individual therapy can be conducted by using cognitive therapy, psychoanalytic therapy, reality therapy, gestalt therapy and behavioral therapy.)
- Assist lead to counsel families as needed regarding addictions issues in accordance with the goals established in the treatment plan.
- Assist lead facilitating group therapy sessions as requested and follows the assigned group format developed for each session.
- Serves as a co-Counselor with other counselors as needed and appropriate.
- Assist lead with preparing progress notes for each consumer and completes the group notes within 24 hours of the initial contact. All individual and family therapy progress notes are to be turned in at the end of the working day of the scheduled session.
- Assist lead with preparation of educational classes as assigned.
- Assist lead with documentation on a group note the clinical observations and behaviors of the client and provides the primary Counselor with a copy of the group note.
- Assist lead in completion of an action sheet on level of a care and assign primary Counselor and provide copies to primary Counselor, case manager, medical records, secretary and house manager (for Residential consumers).
- Attend staff meetings, departmental meetings, seminars/conferences, and service training programs as scheduled.
- Maintains consumer confidentiality according to Federal and The Salvation Army standards.
- Maintains certification or licensure by meeting requirements.
- Enters into the electronic medical records system the activity recordings on consumers and is responsible for changing the staff assignment and level of care as needed.
- All other duties as assigned from time-to-time.
