Title: Children's Care Coordination Intern
Department: Children's Care Coordination
Reports to: Samantha Thompson, Manager
Time Involvement: One semester 8-15 hours per week.
Summary: Working with a care manager, and the program manager an intern would have a firsthand look at the health home program. An internship with the JFS Health Homes would demonstrate the care management process that takes places with every client. The intern would be able to see the assessment and referral process firsthand with the care manager. Conducting home visits, assessments, and referrals to necessary services for clients. An internship would help the intern to gain knowledge about health conditions and navigation of the complex healthcare system.

Completion of Sexual Harassment Prevention Training

Access to a reliable vehicle
Major Responsibilities:
- Assist with completion of paperwork
- Shadow Health Home Care Managers
- Phone calls with clients and home visits
- Working with a diverse client population
- Office tasks as needed
- Navigating health records systems
- Completing Case notes
- Appropriate community referrals
Qualifications: Could be a student receiving and Associates degree, or bachelor’s degree in Human Services or Social Work.
