ASSIST WITH SOCIAL ACTIVITIES - YOUTH AND DISABILITY
Our HACC PYP social programs promote social participation for under 65-year-olds, helping them meet new people and stay connected to their local community.
These programs aim to enhance participants' social, emotional, and physical well-being in a friendly and relaxed group environment, while also providing respite care and support for carers.
Activities are tailored to the needs and interests of participants and include community outings, social meetups, peer support, group activities, and regular care check-in calls.
Your commitment and availability will be coordinated with the Community Support at Home Coordinator.
