This is an ongoing commitment - When required we will give 3 - 7 days notice for appointments. 

Shifts are Monday - Friday   8:00 am - 4:30 pm  

 Staff liaison will provide Wheelchair and Walker Safety -

The care team will share information regarding resident's routine. -

Public Transportation will be arranged ahead of time for the appointments.
