A future where Albertans enjoy more healthcare options in the communities where they live can be strengthened through connections between health services and community partners. The Alberta Cancer Prevention Legacy Fund’s (ACPLF) new initiative, Integrating Health Promotion into Connect Care – Facilitated Clinical Workflows, works collaboratively with healthcare leaders, clinicians, staff, and community based organizations to help Albertans improve their health by making healthier choices easier and by strengthening connections to community supports.
Across Alberta, healthcare teams are already working with the ACPLF to create healthy places and connections for patients and staff. Community partners are creating links between services so that Albertans are able to receive support within their own communities for health goals like quitting tobacco use, reducing alcohol use, and increasing physical activity.
This project aims to provide more support for patients in healthcare settings to achieve their health goals, by creating screening and referral processes for tobacco use, alcohol use and physical inactivity.
Roles and Responsibilities
The role of the Patient/Family Advisor within Integrating Health Promotion into Connect Care – Facilitated Clinical Workflows is to provide the patient perspective related to the development and implementation of this project. Patients are important stakeholders given that a key component of this initiative involves healthcare providers engaging patients in conversations about healthy behaviour actions. A Patient/Family Advisor voice is invaluable in helping identify the most effective way to engage with patients and ensure these interactions are patient centred and culturally safe.
Specifically we are seeking assistance for the following:
- Feedback on acceptability with screening pathways for tobacco, alcohol, and physical activity
- Feedback on patient experience with screening pathways for tobacco, alcohol and physical activity
- Reviewing and providing input on print and educational resources for patients
- Reviewing and providing input on print and educational resources for clinic staff and physicians
- Advising on any additional patient concerns related to the initiative
This project hopes to start April 2022. We anticipate working with the Patient/Family Advisor for approximately 2-4 hours a month for at least 6 months.
All meetings/communication will be held virtually (Skype or Zoom), over the telephone or through email (as appropriate).
Communication and regularly scheduled meeting times typically happen between 9:00 am to 4:00 pm during the week but we can certainly accommodate other times that are more convenient for the Patient/Family Advisor.
