Requests must include:

  1. A completed Application for Internship/Clinical Placement
  2. Personal Resume
  3. Current Transcript
  4. Course Syllabus that includes course expectations & requirements
  5. End of semester evaluation requirements

We reserve the right to deny placement on the premise that we do not have the capacity or time to complete the contractual process.
