Please complete your application form carefully and thoughtfully so that we understand fully why you would like a Work Experience placement with the Trust. Please make sure that you press "Save" at the end of the application form or otherwise your application will not be submitted.
All students wishing to participate in a Work Experience placement with the Trust must complete a Pre-placement Occupational Health questionnaire. This is processed by the Trust's Occupational Health Department. They may contact students if further information is required or if support for students needs to be put in place. In this questionnaire you will be asked to upload your vaccination record. If you do not have your record at home, you can obtain this free of charge from your GP.
We ask all our Work Experience students to enter into an Honorary Contract with the Trust. In this contract the Trust commits to provide high-quality and safe Work Experience placements. In return the Trust requires students to:
- comply with the Mid Yorkshire Teaching NHS Trust's policies and procedures, particularly with regard to Health and Safety at Work
- be aware that all matters relating to patients' diagnoses or treatment are strictly confidential
- behave in a responsible and respectful manner at all times
- follow the Trust's dress policy, including its "bare below the elbows" policy.
We hope that you find this information useful. If we can be of any further help, please do not hesitate to contact us. Our e-mail address is midyorks.workexperience@nhs.net.
