  • I authorize the Corporation of the City of Thunder Bay to contact the persons and organizations listed for the purpose of obtaining reference information.       
  • I permit the Province of Ontario and the Corporation of the City of Thunder Bay and their agents to use any photographs, video tape, or other media taken of me during the 2024 Ontario Winter Games and related activities for purposes of promoting, administering, and managing current and future Ontario Games and related events. I waive claims for compensations for such use.
  • You understand that any breach of confidentiality regarding any information about participants is a cause for dismissal.
  • You understand that you may be required to submit and pass a vulnerable sector screening and/or criminal record check of which information that can be legally disclosed will be made available to relevant parties such as the Province of Ontario, the Corporation of the City of Thunder Bay, the Games Organizing Committee, and appropriate authorities.
  • Personal information contained on this form is collected for the exclusive use of the Province of Ontario, the Corporation of the City of Thunder Bay and the Games Organizing Committee of the 2024 Ontario Winter Games. Information will be used to contact you to volunteer for the event, and if you provided consent, to contact you for other volunteer opportunities. Any personal information the City of Thunder Bay collects is collected under the authority of the Municipal Act, 2001. Personal information is collected in compliance with the Municipal Freedom of Information and Protection of Privacy Act. The personal information collected is for the purpose of maintaining a record of individuals interested in volunteer activities. Please note your contact information may be made available to all  clients of Better Impact that you have selected for your volunteer experience. This information will be made available only to those organizations that you have selected. None of your personal information will be shared, rented, sold or otherwise released to any third party without your consent.
