I have not had permission to undertake paid or voluntary work with children, young people or vulnerable adults refused, suspended or withdrawn in any jurisdiction, and I have not engaged in behaviour that would justify any such withdrawal; and

I have disclosed to SSI Group any actual, potential or perceived conflicts of interest that may be relevant to my volunteer role with SSI Group, and have agreed with SSI Group on how they will be managed.
