To be a member of the MPTF Student Volunteer team, please review and agree to the following requirements for volunteering.

  • Commitment to schedule
  • Parental Consent Form (can be completed after orientation)
  • Liability & Photo Release Waiver (can be completed after orientation)
  • Confidentiality Statement and Agreement (can be completed after orientation)
  • Compliance with current MPTF health clearance guidelines, TB test and flu vaccine are mandatory
  • Maintaining the privacy of our clients and residents is important. That means you may not share any names or personal information about residents or clients with friends or family or on any form of social media.

We expect all MPTF Volunteers to act and dress in a respectful and professional manner.
