Consent and Photo Release
By his/her signature below, the participant understands that volunteering at Top Box Foods may involve working in warehouse conditions and can sometimes include, but is not limited to, lifting, working around heavy moving equipment, and handling food products including products containing peanuts and tree nuts.
The participant also understands that our warehouse includes refrigerated and freezer spaces and therefore will dress appropriately. Appropriate dress for volunteering in the warehouse includes closed-toe sneakers or work boots, long shorts or pants, and winter gear. In the case that the participant will be out on deliveries, he/she understands that they should to take care to dress appropriately for the day’s weather.
By his/her signature below, the participating individual also understands that they are expected to follow safety rules and any other rules and voluntarily agrees to assume and/or incurs all risks of loss, impairment, damage or injury of whatever kind, including death, that may be sustained or suffered by participation in this program whether or not the result in whole or in part of acts or omissions, negligence or other unintentional fault of Top Box Foods. In addition, the participant (including his/her heirs, assigns and personal representatives) agrees to release, hold harmless, and indemnify Top Box Foods or its affiliates from and against any claims, demands, actions, causes of action, lawsuits, expenses, or losses (including attorneys’ fees) on account of property damage or personal injury (including death) arising out of or attributable to the individual’s travel to or participating in our program.
This Assumption of Risk and Release Waiver applies to Top Box Foods and all of its trustees, officers, directors, managers, agents, staff, volunteers, advisers and/or representatives.
For Minors:
Every individual under the age of 18 must have parent/guardian consent prior to volunteering with Top Box Foods. In the event of an injury, the parent/guardian authorizes Top Box Foods staff to seek treatment for minor volunteers and to take other action should a medical emergency arise and to waive and release any right for damages. Volunteers are expected to follow safety rules and all other rules related to the warehouse and on delivery days. Top Box Foods accepts no liability for minors.
Photo Release:
The participant hereby gives Top Box Foods permission to copyright and/or use, reuse and/or publish and/or republish pictures or images of me for the purpose of illustration, advertising, and promoting Top Box Foods through any medium.
Personal Vehicle:
The participant understands that if they are using their own vehicle while volunteering at Top Box Foods, that vehicle is insured. The participant agrees that Top Box Foods is not responsible for any damage to personal property, including the vehicle used while volunteering.

Health and Safety during the COVID-19 crisis:
The participant agrees that they have been in good health for the past 14 days.
