APPLICATION FORM INSTRUCTIONS

The information that you provide will be used to assess your suitability for a volunteering opportunity with disAbility Living. It is therefore in your best interest to complete all sections of this form accurately.

Please ensure you have read the Volunteer Handbook before submitting a volunteer application form.

PRIVACY STATEMENT

The personal information that we collect via this application is used to assess your suitability for a volunteer position, and is only assessed by relevant staff members in the performance of their duties. The information is held in confidence by disAbility Living except where the release of such information is required by law. No information is disclosed to other organisations or persons outside of disAbility Living except with the written permission of the applicant.

VOLUNTEER REQUIREMENTS

Volunteers do not need any previous experience in volunteering or with people with a disability to fulfil any of our volunteer roles and they do not administer medicine, food or drink nor provide any personal care.

Please note that in order to volunteer with disAbility Living you may need to fulfil the following requirements before commencing volunteer duties:

Note: We will assist you in obtaining any training and screening checks required for your role. Any approved out of pocket expense will be reimbursed.

Role Requirements and Training Opportunities:

Essential training includes:

  • NDIS ‘Quality Safety and You’.
  • Supporting People to Stay Infection Free (COVID-19), and an
  • Induction.

Some roles may also require:

  • First Aid and CPR, and
  • Child Safe Environments: For volunteers working with children.

disAbility Living will discuss the required training with you and can assist with training requirements. Volunteers will be reimbursed for any approved out of pocket training expenses.

 

 

Screening Checks:

Essential screening checks include:

  • A Disability Employment Screening Check.
  • A Working with Children Check: for those working with children.
  • 2 Reference checks, and
  • An interview.

disAbility Living will assist with the screening process, which is free for volunteers.

Other Requirements:

Comprehension of the Volunteer Handbook and the Volunteer Agreement. These provide information about disAbility Living and the Interchange Volunteer Program including; volunteer rights and expectations, health and safety, policies and procedures, expenses and reimbursements, transporting clients, insurance, confidentially, photograph and image consent, complaints process and mandated reporting.
