Volunteer Agreement

  1. I agree that I am a professional in good standing with my regulatory college/ body, and will continue to be so upon my acceptance to Canadian Medical Assistance Teams (CMAT), as a registered health professional or paraprofessionals in a province or territory in Canada.

  2. I agree to behave in accordance with my professional/ paraprofessional college or body's Code of Ethics and Standards of Practice at all times while deployed nationally and/or internationally as a CMAT Volunteer.

  3. I agree to submit a Criminal Record Check with vulnerable sector screening to CMAT. Click HERE for more information on Criminal Record Checks.

  4. I confirm that I have not previously been rejected when applying to Canadian Medical Assistance Teams, nor have been dismissed previously as a CMAT Volunteer.

  5. I certify that the information provided in this application is true and correct. I understand that withholding of information or giving false information will result in immediate disqualification of volunteer candidacy, and/or in disciplinary action up to and including involvement of my Professional Regulatory College/Body.
