Thank you for your interest in volunteering at The Dudley Group NHS Foundation Trust via the new DGFT Advance project - youth volunteering in healthcare. Please note that we are unable to offer volunteering to under 18 year olds outside the hours of 7.30am-4.30pm weekdays.
Any voluntary placement, if offered, will be subject to the information supplied by you as being correct. You must understand that if you knowingly withhold valid information or include incorrect information your voluntary placement is liable to cease. You must also understand that any placement will be subject to satisfactory references, health clearance and satisfactory police check appropriate to the placement area. In addition, you understand and agree that by completing this form you give consent to the information being held and processed by the Trust in relation to your application/volunteer placement in accordance with current data protection guidelines. You retain the right to withdraw your consent at any time.

Important: You should not apply to volunteer if you or your family are concerned about the risks of volunteering in a hospital setting as any social contact carries risk. You must also be aware that due to current Government guidelines you would be required to wear a surgical face mask at all times whilst in the hospital.
