This Personal History Statement will be used as a basis for a background investigation. The information will be used as an aid is assessing your suitability to volunteer with our Office. Therefore, it is important that the questionnaire be filled out completely, accurately and legibly. All statements are subject to verification. The information you provide will not be the only factor for determining whether you will be allowed or denied the opportunity to volunteer at Yolo County Animal Services.

The Uniform Electronic Transaction Act (Civil Code section 1633.5, et seq.) provides that electronic signatures can be legally binding. By completing and answering questions in this application, you are acknowledging and agreeing that you are submitting your responses in an electronic format which will be accepted your electronic signature.
