WHS Group Volunteer Application Form Agreement
Thank you for your group's interest in volunteering with The Winnipeg Humane Society! If you have a MyImpact account through another organization you volunteer with, please enter your username & password. If you do not, or are unsure, please first set up your MyImpact account.
Join our awesome volunteer crew by first reviewing and then agreeing with the policies below related to your application form.
1. I am at least 18 years of age and our team members are all at least 14 years of age in order to be considered for group activities. I will ensure that appropriate supervision will be provided for anyone that is under 16 years of age.
2. I authorize the WHS to collect and retain my personnel information in connection with my group's voluntary activities.
3. I understand that further documentation pertaining to The Winnipeg Humane Society's policies and procedures in a form of a Volunteer Agreement and/or Volunteer Waiver. I will review these documents and then submit my authorized form.
4. I understand that providing false information on this application may prohibit me from volunteering with The Winnipeg Humane Society.
