I understand that Lakeridge Health Volunteer Resources:

Can only contact me if my application is being considered for current volunteer opportunities.
Upon acceptance, will do their best to place me based on available opportunities and my interests and availability.
Upon acceptance and completion of all onboarding requirements, will provide orientation, training, and guidance.

I agree to:

Be at least 16 years old when I submit this application
Complete all onboarding requirements, including a mandatory health screening (two-step TB test and physician confirmation of immunizations)
Provide a police record check, including Vulnerable Sector Screening, if requested
Commit to at least one 3–4-hour shift per week for a minimum of six months
Adhere to all Lakeridge Health and Volunteer Resources program and policy requirements including annual training
