Our liability coverage requires all CSBL volunteers to be registered and sign in before each shift.

This form is ONLY for volunteers 13-17 years old

INJURY RELEASE:

A Parent/Guardian Release For Volunteers Under 18 is needed to participate.

VOLUNTEER POLICIES:

CSBL may use any photographs, motion pictures, recording or any other media of this event for legitimate purpose.
CSBL has the right to terminate participation of any volunteer without cause or explanation, and CSBL shall have no liability therefore.

By agreeing to this form I am indicating that I have read and understood this consent form and agree to the Volunteer Policies as stated above. This form will remain in effect until CSBL or I terminate my participation

We do not share your personal information and will not use your email address for any purpose other than communicating about Christmas Storybook Land activities.
