Welcome to the Youth and Family Advisory Council (YFAC) Volunteer page! Please read the following statement before moving forward to the application.
If appointed as a Youth and Family Advisory Council member, I agree to:
- I am a youth member between the ages of 16 and 24 OR a caregiver/family member of current or former CASA patient. At this time our youth seats are inteded for youth who have attended CASA programs.
- Serve my full three year term.
- Attend monthly meetings as expected. See Terms of Reference for details.
- Inform the Youth and Family Advisory Liaison by text or email as far in advance as possible if I am not able to attend a meeting or need to attend virtually.
- Treat others with respect and come forward if I think I have been treated unfairly, and support others to do the same.
- Adhere to the procedures and policies in the Youth and Family Advisory Council's Terms of Reference (you can view and sign the document before submitting your application).
- Adhere to CASA's Code of Conduct (you can view and sign the document before submitting your application).
- Adhere to CASA's Confidentiality Agreement (you can view and sign the document before submitting your application).
I declare the information filled in this application is true and complete. I understand that any false information provided may be cause for denial of a volunteer placement or dismissal after placement and my volunteer status may be immediately revoked by CASA at its own discretion. This information will be used to process my eligibility for a suitable volunteer position.
