Welcome to the Youth and Family Advisory Council (YFAC) engagement portal! Please ensure you meet the following eligibility criteria before completing the rest of this application:

  • I am a youth member between the ages of 16 and 24 OR a caregiver/family member of current or former CASA patient. 
  • I am able to serve a 1,2, or 3 year term on the council.
  • I have the capacity to attend monthly in-person meetings at CASA centre in Edmonton. See Terms of Reference for details. 

*** Please note that we are only accepting applications for family members at this time, all youth seats on the council are currently filled.****

I declare the information filled in this application is true and complete. I understand that any false information may be cause for denial or dismissal after placement from the Youth and Family Advisory Council and my sitting member status may be immediately revoked by CASA at its own discretion. This information will be used to process my eligibility the Youth and Family Advisory Council.
