I understand that Peninsula Home Hospice is unable to accept all applications for volunteering.

I understand that due to the nature of volunteering with Peninsula Home Hospice, I may not currently be suitable to volunteer if I have had:

A close friend/relative die in the last 12 months
A recent significant life challenge or life change

I am interested in volunteering with Peninsula Home Hospice and can commit to volunteering for at least one year, if accepted.

I agree that the information provided in this application is true and correct and if accepted as a volunteer, agree to follow all guidelines and policies.
