Volunteer Commitment

I am willing to make a commitment of my time and agree to serve as a volunteer with Miracle City Hospice, an Addus Homecare company. I understand that my commitment is of vital importance to continuity of the Hospice program and allows us to build trusting relationships with our patients. I have read my job description and understand it. I understand that any patient/family information to which I have access to is privileged and shall be held in strict confidence. Patient/family information will only be shared with appropriate hospice personnel.

I pledge to:

Be punctual and conscientious in carrying out responsibilities in reporting and record keeping.
Keep all information confidential.
Endeavor to keep work standards high in order to uphold the quality of our program.
Work with consideration and courtesy.
Support the dignity of our patients.
Promote the hospice concept in our community.
Inform the Volunteer Coordinator of any inability to meet my commitment at the earliest possible date.
