Application Instructions

READ COMPLETELY:

You must complete an application by creating a username and password in the "I am new to MyImpactPage.com" section at the bottom of this page.

Students must be 16 or older by June 30, 2025. Students should plan to volunteer a minimum of 56 hours (2 four-hour shifts per week), over at least 7 of the 8 weeks, to receive their certificate of completion at the end of the summer.

Shifts are Monday through Friday- 8:00am-12:00pm and 12:00pm- 4:00pm. We do not have volunteers under the age of 21 at the hospital at night or on weekends.

Once you complete the application form, please click the submit button at the bottom. We will send you an email to confirm we have received your application.

You will need to enter a unique username to identify yourself to the system. You should select something that is easy for you to remember such as your email address or your name. Your username must be at least 6 characters long. If the name you enter is already in use by someone else, you will be prompted to choose another username.
