Waiver & Release of Liability Form for Youth Volunteers (14 to 17)
This waiver is for youth volunteers ages 14 through 17 who volunteer without a parent or legal guardian present.
Youth volunteers at the MetroParks of Butler County sometimes agree to participate in activities that may involve some risk. As with any volunteer, they always have the right to refuse any task that they feel is unsafe or for which they are not interested. Youth volunteers agree to follow the safety directions of the MetroParks staff and exercise reasonable care in all activities in which they participate. Additionally, the MetroParks staff will take every reasonable precaution to ensure the safety of youth participating in the volunteer program and to carefully explain each assigned task. Youth volunteers will not be asked to, or permitted to, operate equipment or take part in any activities which, if they were employees, that Ohio Minor Labor Laws prevent their participation.
MetroParks of Butler County photographers may document the programs and events. MetroParks of Butler County reserves the right to use photographs of participants taken for the purposes of advertising and promoting its programs. The activities that youth may participate in at MetroParks include, but are not limited to litter cleanups, naturalist programs, painting and staining projects, trail maintenance, farm animal care, program preparation, and special event tasks.

I have read the above and agree to allow my child to partake in the activities mentioned above and agree to assume the reasonable risk of participation in these activities.
Waiver & Release of Liability Form for Volunteers (13 and younger)
This waiver is for adult volunteers (ages 18 and older) as well as youth volunteers ages 13 and younger volunteering with a parent or legal guardian present throughout the volunteer activity.
I understand that I am not an employee of MetroParks of Butler County and that any duties I perform are as a volunteer. I understand that I will not be reimbursed for mileage or any other expenses. I agree to abide by the procedures and bylaws set forth by MetroParks while performing my assigned work duties. If applying for a position that requires a background check, I understand I will be notified of this fact and will be required to give the MetroParks permission to have this done. I recognize there are certain risks of injury as a result of my (or my child’s) participation in this volunteer activity. I agree to assume the full risk of any injuries, damages, or loss which I (or my child may) sustain as a result of participating in any and all of the activities connected with or associated with this program, or products provided, including allergic reactions to foods consumed. I agree to waive and relinquish all claims I may have, as a result of my (or my child’s) participation in this program, against MetroParks of Butler County and their agents, employees, and volunteers and against any co-sponsors of the program. I understand that neither MetroParks of Butler County, its staff, nor its volunteers assume responsibility for accident or injury to participants during this activity.
