Volunteer Statement
By accepting these policies I am acknowledging that I am applying to be a volunteer who chooses to act in recognition of a need without concern for monetary profit. Through my volunteer work, I enhance the mission of Swiss Village, Inc. and wish to donate my time. I will provide courteous services to all residents, children, community members, staff, visitors and other volunteers and interns. I will abide by all rules, regulations, policies, and supervisor's instructions. I certify that all statements made in this application are true and correct. I agree to complete or allow Swiss Village, Inc. to perform: a criminal history check completed under the Fair Credit Reporting Act, Office of Inspector General Medicare/Medicaid exclusions check, sex offender registry check, TB Test, fingerprinting, physical, and/or CPR certification if needed for my volunteer position.
Privacy Practices Statement
As a volunteer at Swiss Village, Inc. (Swiss Village, Kinder Haus, and the Muselman Wellness Pavilion) I understand that it is important to keep all information private. All information learned while volunteering is considered confidential. If information needs to be discussed with staff, it should be done so in a private setting with your supervisor or the Director of Volunteers. I understand that it is my responsibility to safeguard the security of residents' health information, along with information pertaining to residents, children, and community members at all facilities. I acknowledge by accepting these policies that I will abide by Swiss Village's Privacy Practices, HIPAA Regulations, Code of Conduct, and Residents' Rights policies. I understand that a violation of these policies may result in disciplinary action, up to and including termination or legal action.
Liability Waiver
While volunteering at Swiss Village, Inc. (Swiss Village, Kinder Haus, & the Muselman Wellness Pavilion) I understand that this waiver discharges Swiss Village, Inc. from any liability or claim that I may have against Swiss Village, Inc. as a volunteer. I also understand that Swiss Village, Inc. does not assume any responsibility for or obligation to provide financial or other assistance including but not limited to medical, health, or disability insurance in the event of injury or illness.
