The applicant understands that only volunteer opportunity in Ontario is through this online application.
The applicant understands that this application is for volunteering as an individual. If you are applying as a group from a community/company/faith organization; please visit AgeCare's volunteer page and select Apply Now - Group.
The applicants understands that this is not an application for a practicum or work experience placement. Please contact the AgeCare community directly to learn more about these experiences.
The applicant agrees that they will abide by all policies and procedures of the AgeCare volunteer program.
Volunteers must be 14 years or older with Ontario AgeCare communities.
Volunteers 18 years and older require a Vulnerable Sector Search (VSS) and Criminal Record, completed within the past 6 months. Volunteers understand their need to procure one upon acceptance into the volunteer program (this is explained within the interview).
The applicant understands that the information provided in the application will be used to process their eligibility for a suitable volunteer position. They declare that the information provided on the application will be true and complete. The applicant understand that any false information provided may be cause for denial of a volunteer placement or dismissal after placement.
The applicant understands that a volunteer role with AgeCare does not constitute a guarantee or promise of future employment, nor do they entitle the Volunteer to greater consideration for any future employment or volunteer opportunities.
If a volunteer has or currently works or volunteers with any AgeCare community, as either an employee or with an agency, this is identified on the application and resume.
The applicant authorize AgeCare to contact individuals and organizations named on this application to obtain further information that would assist with their placement/success as a volunteer.
The applicant consents that AgeCare may collect, retain information and conduct further investigations with respect to the information. The volunteer further consent to the updating of this information as necessary.
The applicant understands volunteers are required to complete a 3 month / 50 hour probation period from the date of their first shift (following onsite orientation).
The Volunteer consents to the collection, use and disclosure of all personal information contained in this Agreement, and otherwise collected by or on behalf of AgeCare for all purposes in accordance with the Personal Information Protection Act (PIPA) and Personal Information Protection and Electronic Documents Act (PIPEDA).
I understand that proceeding with the application confirms that I have read the above and agree to abide if accepted as an AgeCare volunteer. I also agree not to disclose confidential information of any nature after I cease volunteering in this capacity.
