Volunteer Waiver, Background Check Permission, and Policy Agreement

Volunteer Policy
Children's Museum of Sonoma County’s policies are kept up to date in our Volunteer Handbook, which can be downloaded here. Volunteers can expect to be treated with respect and gratitude and should treat others this way. We strive to provide fulfilling volunteer experiences to all members of the community who support our mission. Due to our work with children, all recurring volunteers 18 and older must pass a background check before they start volunteering.
Background Check Authorization (for volunteers 18 years and older)
I authorize the Children's Museum of Sonoma County to obtain and rely upon Consumer reports or investigative consumer reports in considering me for volunteer employment and, if I am employed, in considering me for subsequent promotion, assignment, reassignment, retention, or discipline. By my signature below, I authorize the Children's Museum of Sonoma County to obtain any such reports and to share the information received with any person involved in the employment decision about me.
I authorize you to contact my current employer for Employment and Reference Verifications.
I also agree that this Disclosure and Authorization in original, faxed, photocopies, or electronic (including electronically signed) form will be valid for any consumer reports or investigative consumer reports that may be requested about me by or on behalf of Children's Museum of Sonoma County.
I have the right to make a request to IntelliCorps Record, Inc, upon proper identification, to request the nature and substance of all information in its files on me at the time of my request, including sources of information, and the recipients of any reports on me which IntelliCorp Records, INC has previously furnished within the two-year period preceding my request.
I certify that all elements of the personal data I will provide are true, accurate, and complete. I understand and agree that any omission, false statement, misleading statement, or answer made by me on my application or any supplements to it and in any interviews will be sufficient grounds for rejection of employment and my discharge after employment.
Volunteer Waiver for Volunteers
Release from Liability
By continuing this application, you waive your right to hold the Children's Museum of Sonoma County (CMOSC) and its officers, employees, other volunteers, independent contractors, and representatives (the “Museum People”), liable for any injury or loss suffered by you during volunteer work with the Museum.
Image Release
Finally, by continuing this application, you grant to CMOSC and its staff the rights to use video and/or photo images of you for marketing, social media, flyers, presentation slides, and any other promotional uses undertaken by CMOSC. I waive any right to inspect or approve the finished images or any printed or electronic matter that may utilize them, or to be compensated for them.
By continuing this application, you attest that you have never been convicted of any type of sex crime, crime against children, kidnapping/abduction, crime related to aggravated assault or alcohol abuse, drug-related crime, or hate crime, nor have you been ordered by the court to receive psychiatric or psychological treatment in connection therewith.
All youth volunteers under 18 years old MUST obtain a parent/guardian signature in order to participate in the youth volunteer program at the Children’s Museum of Sonoma County. This signature must be on file before the volunteer starts volunteering.
