Volunteer Agreement
AGREEMENT BY VOLUNTEER:
I agree to volunteer my services to The Community Library, Gold Mine Stores, Wood River Museum of History and Culture, and Book Mobile (hereafter identified as “The Library”) in accordance with the following understandings:
I will offer my services with no expectation of pay, benefits or other privileges of employment of any kind. I certify that I have not been promised and have no expectation that I will receive a paid position as a result of my volunteer work. I acknowledge that I am not currently an employee of The Library.
I understand that placement into volunteer positions are at the sole discretion of The Community Library. Accordingly, my volunteer assignment/service may be withdrawn or terminated at any time. I understand that theft or inappropriate removal or possession of property of The Community Library, its entities, staff, volunteers or patrons will lead to my dismissal.
I acknowledge that The Library retains the right to require a background check as part of the volunteer application process and will require my consent to conduct the background check.
I agree that I will protect confidentiality of the Library’s patrons, donors, staff or other constituents (names, addresses, social security numbers, physical disabilities etc.) should be revealed to anyone outside of the Library and only to those in the Library to whom the information is necessary for the completion of their duties.
My volunteer service will not confer on me the status of an employee of The Community Library, while acting within the scope of this Agreement.
I certify that if approved to Volunteer at The Community Library I will be given and will read The Community Library Volunteer Handbook. I understand my volunteer duties and the potential hazards/risks that are involved, if any, for this volunteer position. I understand it is my responsibility to be aware of my physical condition and I will not engage in physical activities that are beyond my capabilities. I will notify my volunteer coordinator/supervisor of any significant change in my ability to do the work. If special skills are required for this project, I acknowledge I have been appropriately informed of the required tasks.
I grant The Community Library permission to use my image in any and all publicity, advertising, and other materials, including websites and other electronic forms, or any other uses The Community Library may deem proper. I waive any right to payment, royalties or any other consideration for use of the image in perpetuity.
I hereby hold harmless and release and forever discharge The Community Library, its officers, agents, employees and volunteers from all claims, demands, and causes of action which I, my heirs, representatives, executors, administrators, or any other persons acting on my behalf or on behalf of my estate have or may have which arises out of or is in any way connected with this agreement.
If I drive a personal vehicle to and from a volunteer project, I certify that the vehicles are properly insured as required by Idaho State Law.
I understand and agree that The Community Library is not responsible for any of my personal property that is lost, damaged or stolen while I am participating as a volunteer.
As the parent, guardian, or supervising adult of the applying volunteer (if under the age of 18) I have read The Community Library Volunteer Handbook, agree to all The Community Library’s Volunteer Policies found in the Handbook on their behalf, and grant permission for my child or ward to perform volunteer services for The Community Library. I will keep informed of any changes in The Community Library’s Volunteer Handbook that may affect their service or changes to their service requirements.
If representing a group, such as a church group, school group, corporation, civic club, or other organization, it is my responsibility to ensure that all group members are given access to The Community Library’s Volunteer Handbook and are informed of the Library’s Volunteer Policies. By agreeing to the Library’s Volunteer Policies, I acknowledge that I have informed all volunteers from our group of the Library’s Volunteer Handbook and Policies and they have all agreed to the Library’s policies as a prerequisite of their volunteer service. It is my responsibility to keep my group’s membership informed of changes in the Library’s Volunteer Handbook or changes to their service requirements.
If I am completing community service (school, court-ordered, or club related), I will follow the guidelines in the Volunteer Handbook for recording and documenting hours of service.
