Am in grade 6 to 12
Will notify the library in advance if I must change my schedule or miss any shifts (if applicable)
Will follow guidelines and comply with library policies at all times
Allow Naperville Public Library may use my likeness in any materials (photos, videos or voice recordings) that will be used to promote Naperville public Library
Release and hold harmless the Naperville Public Library, its officers, directors, agents and other employees, from any and all claim of whatever nature that I might have now or in the future as a result of my participation as a volunteer for the Naperville Public Library
Certify that all statements on this application are true and correct to the best of my knowledge
