Consent
IN CONSIDERATION of the acceptance and the permission to participate as a member of Mon Sheong Youth Group, I, for myself, my heirs, executors, administrators, successors, and assigns HEREBY RELEASE, WAIVE AND FOREVER DISCHARGE Mon Sheong Foundation, Mon Sheong Long-Term Care Centres, Mon Sheong Youth Group, Mon Sheong Foundation Chinese School and other events referred by Mon Sheong Foundation, OF AND FROM ALL claims, demands, damages, costs, expenses, actions and causes of action, whether in law or equity, in respect of death, injury, loss or damage to my person or property HOWSOEVER caused, arising or to arise by reason of my participation in any activities in the said locations. Furthermore, I understand that the Mon Sheong Foundation will use my personal information as required for any event and hereby release any claims against the Mon Sheong Foundation for any matters in relation to the Privacy Act.
CONSENT: The undersigned consents to the collection and use of the information provided above by the Mon Sheong Foundation and Mon Sheong Foundation Youth Group and to any photographs, motion pictures, video tapes, laser disc and/ or audio recordings taken of the above member, for the promotions and other purposes.
