Vaccination Policy:
COVID-19 Vaccination Policy (excerpts from REF# 3428 v.8)
Effective November 1, 2021 all Avow staff members (employee and volunteer) were required to be vaccinated against COVID-19 by having completed a primary vaccination series, unless otherwise exempted from this policy by an approved accommodation pursuant to the interactive process. Staff requesting exemption to this policy should reach out to the Human Resources department. Avow considers staff fully vaccinated if it has been two weeks or more since they completed a primary vaccination series for COVID-19. Avow will require staff and contractors servicing Avow patients, receive timely and appropriate COVID-19 boosters as required by CMS. For any period of time that an employees or contract staff member do not meet the definition of fully vaccinated as set forth in this Policy (e.g., staff member has been granted an exemption from vaccination as an accommodation), the individual must continue to abide by masking and social distancing requirements including, but not limited to, weekly or more frequent COVID-19 testing, wearing a N-95 mask over the nose and mouth at all times while on duty regardless of the workplace setting, ensuring 6-feet distance between non-household family members, regularly cleaning and disinfecting workplace surfaces, as well as monitoring and reporting daily health symptoms (e.g.,fever, cough, shortness of breath, sore throat, irregular temperature, etc.).
Annual Influenza vaccination is required for all staff.
