PEER-LED SUPPORT GROUP HOST AGREEMENT
VOLUNTEER AGREEMENT
This AGREEMENT is made between RESOLVE: The National Infertility Association (hereafter called "RESOLVE") and all individuals applying to volunteer as a "Peer-Led Support Group Host" (hereafter called "PLSG Host").
RESOLVE is a tax-exempt, nonprofit corporation, incorporated in the state of Massachusetts, dedicated to providing information, education, advocacy, and support for individuals experiencing infertility and raising public awareness about infertility.
The PLSG Host is an individual who voluntarily supports RESOLVE's mission by leading a support group to further RESOLVE's goals. The PLSG Host acknowledges that they are not an employee or agent of RESOLVE and will receive no compensation or financial benefits from RESOLVE. The PLSG Host does not have the authority to, and shall not, take any actions or sign any documents that serve to bind RESOLVE and shall not hold themselves out as having the authority to make any commitments on behalf of RESOLVE.
TERMS AND CONDITIONS
- Volunteer Period: This Agreement shall remain in effect as long as you serve as a peer-led support group host volunteer for RESOLVE or until terminated by RESOLVE. This Agreement shall be reviewed and may be renewed at the discretion of both parties each year or earlier as circumstances determine.
- Duties of the Peer-Led Support Group Host: The PLSG Host shall comply with each of the administrative guidelines set forth in the PLSG Host Manual, including subsequent modifications issued by RESOLVE, as follows:
- Meeting Introduction: The PLSG Host must read the RESOLVE Peer Group introduction at the beginning of each meeting. In addition to providing contact information for RESOLVE staff, if requested.
- Support Group Communications: The PLSG Host agrees to communicate proactively with prospective and current group members to provide updates on support group details and scheduling.
- Email Signature: RESOLVE Volunteers should use the following email signature when communicating with support group members. Personal business, company, or any other affiliated signatures are not to be used in communications.

[Support Group Directory link] - Support Group Marketing: The PLSG Host is responsible for regularly promoting their support group virtually and in-person.
- Branding and Listing
- Use of RESOLVEās Logo: The RESOLVE logo must be used when listing support groups both virtually and in hard copy.
- Support Group Description: Support groups must be described as "RESOLVE support group(s)" and should only link to RESOLVE sign-up forms located in each listing in the resolve.org Support Group Directory.
- Contact Information: The PLSG Host agrees to provide RESOLVE with updated contact information. This information will be used by RESOLVE staff for communications and will be shared with prospective group attendees and publicly through our website resolve.org.
- Group Sign-Up: The PLSG Host agrees to request and direct support group participants to sign up for their support group via RESOLVE sign-up forms on the resolve.org Support Group Directory. These forms provide liability waivers and guidelines, offering protection to RESOLVE staff and volunteers and setting expectations for participants.
- Feedback Reports: The PLSG Host agrees to report about their support groups monthly, even to report when a support group does not take place or has zero participants attend. RESOLVE will periodically request feedback reports from support group hosts to determine the stability of the support group and address any concerns.
- Training: The PLSG Host agrees to attend virtual training sessions for peer-led support group hosts as they are available. Taking part in training ensures you are updated with policies and procedures that is a fundamental part of your role as a volunteer with RESOLVE.
- Professional Conduct: The PLSG Host agrees to avoid any acts, comments, or inferences that might injure, directly or indirectly, the professional reputation of RESOLVE, its Board of Directors, employees, agents, or professional members.
- Indemnity: The PLSG Host agrees to hold RESOLVE harmless from any losses due to events or comments made by any member of the peer group.
- Representation: The PLSG Host agrees they are not representing any company, individual business, or profitable venture. RESOLVE support groups must not be used to promote personal business interests or professional careers. Hosts may disclose their professional background if it is relevant to infertility and can provide valuable insights to the group, but they must not use the group to solicit clients or promote their business. Mental health professionals interested in starting a professionally-led support group should email info@resolve.org.
- Group Member Dynamics: The PLSG Host agrees to notify RESOLVE staff at supportgroups@resolve.org if they experience any problematic group members or members needing mental health assistance. RESOLVE staff can assist hosts in providing the resources needed for individuals experiencing a mental health crisis or those deemed not a right fit for the support group.
ACKNOWLEDGEMENT
I acknowledge that I have read this Agreement in its entirety. Furthermore, I attest that I do not have any actual or potential conflicts of interest to disclose at this time; alternatively, I have completed the attached form disclosing any actual or potential conflicts of interest that exist at this time.
By completing this agreement, I agree to the terms and conditions set forth in this Volunteer Agreement.
