Thank you for volunteering for a Corporate Volunteering Day for Demelza.
Please read the following information before completing your application form.
Important information:
• Please wear appropriate clothing for your role
• Treat everyone equally, respectfully and do not discriminate
• Do not use offensive or inappropriate language or behaviour
• Inform a member of staff of any safeguarding concerns
• Report any accidents or incidents to a member of staff
• Ask for donations in a polite manner
• To sign in & out when volunteering for Demelza where necessary
• Please ensure your mobile phones are on silent when volunteering with us.
• Make yourself aware of the designated fire exits and assembly points.
• Lift objects safely and only when necessary.
• Not to disclose to anyone outside of Demelza any information about the children,
families, staff, volunteers or supporters.
• Adhere to the Data Protection Act 2018 in line with Demelza policy.
• Be aware of the Demelza Smoking Policy - All Demelza sites are “Smoke-Free Zones” in line with Government legislation (including e-cigarettes and vapes). There may be a nearby designated smoking area and you should speak to your Demelza Lead about where this may be located for the site you are volunteering at.
• Follow the Demelza safeguarding policies and procedures. Information will be given in your helpful guidelines you will receive once applying.
Please note: we will take photos of volunteering groups for social media posts, internal comms and our corporate partners’ use. Taking part in our volunteering sessions gives us consent to do so, UNLESS you opt out of photos on the day.

Age and Insurance:
All volunteers need to be over the age of 16.
A Risk Assessment will be completed with any volunteers who are under 18.
All volunteers are covered under our Public Liability Insurance.

Demelza retains on file information from this form in accordance with the
General Data Protection Regulations. Access and storage of data is
managed in accordance with our Privacy Policy, which can be accessed
