Commitment Requirement Agreement

This is the first step to apply to become a volunteer for Hawthorne Seniors Care Community.

We require a Criminal Record Check, interview, orientation, proof of vaccination status (2 doses of covid-19), & Masks must be worn on site

Please note the Volunteer Coordinator is happy to provide a letter of reference but will do so once the Volunteer has fulfilled their commitment.
