Please follow the instructions below to apply to become a member of Faithful Paws Pet Therapy OR to login to the membership system. If you are applying to become a member, please remember to have your documents ready before creating a username below. Refer back to this page on our website for the list of documents that are required as part of the application process.
- Complete the information on the LEFT side of the screen below (under "I am new to MyImpactPage.com"). You will create a username, enter and verify your email address, and check the box to indicate that you understand the application process. Click Save and Continue.
- Then, you will be on the Contact Information screen. Please complete all required fields using your PET'S name in the First Name field and your last name in the Last Name field.
First Name = Fido (your pet's name)
Last Name = Smith (your last name)
Complete this information for ONLY ONE PET. You will be able to add more pets later should you wish to enroll them in the program.
- Click Save and Continue.
- The third screen will ask you about your availability, how you heard about Faithful Paws, information about your pet, and information about you. It also asks you to acknowledge and agree to important policies. When you have completed all of the information, please click "Submit Application."
- You will land on a page that says "Application Complete." Please click on "HOME" to upload the required documents, add a second handler (if desired), and pay your membership fee. Or, if you are not ready to upload documents, you may logout.

- Login on the RIGHT side of the screen below by clicking on "Go to volunteer login" at the VERY bottom of the screen.
- Click "MY PROFILE" in the top right to access the Contact Information and Additional Info screens.
