Volunteer Career Guide Application

Volunteer/Student Statement:

I understand that the Niagara Folk Arts Multicultural Centre (NFAMC) will be collecting, using and disclosing my personal information for the purpose of establishing and managing a volunteer/co-op relationship.

I also consent to the collection and use of my personal information in order to ensure the safety of NFAMC clientele, for statistical purposes and to inform me about NFAMC programs or services as well as important Volunteer/Co-op Program updates.

If I no longer want to be on the current volunteer/student email list, I understand that it is my responsibility to make this request to my Volunteer Coordinator.

Privacy Contract:

I, the undersigned, do willingly affirm that I will hold in confidence all matters that come to my attention while serving as a volunteer/student at NFAMC. I will respect the privacy of clients, staff and all other persons with whom I come into contact over the course of my volunteer work/placement at the Centre. I agree to honour and follow all established confidentiality and conflict-of-interest policies, guidelines and procedures of NFAMC.

If I am in doubt about the nature of confidential information or have any questions about anything in the Centre's Code of Ethics Policy, I agree to discuss these matters with my Volunteer Coordinator.

Program Requirements:

In my volunteer/student capacity at NFAMC, I agree to the following three requirements:

• I will bring any conflict, issue or disagreement to the attention of my Volunteer Coordinator immediately.
• I will assist the newcomer client only in public spaces and understand that meeting in each other’s homes is not allowed.
• There will be no monetary exchange of services provided.

I have also read and agree to the full list of Volunteer/Co-op Program Requirements at NFAMC and understand that if I am unsure about any of these requirements that it is my responsibility to ask my Volunteer Coordinator for clarification.

Photo Release:

I hereby authorize the taking of photographs or videos by staff, employees or partners of NFAMC for the use of:

• Educational/Training Purposes
• Media Release
• Publicity (pamphlets and posters)
• Social Media (Facebook, Twitter, Instagram and Website)

I also authorize NFAMC to include my name in the credits of my image.

If I do not consent to having my photo taken and/or having my name included with my photo, I understand that I must inform my Volunteer Coordinator immediately.
