Confidentiality Policy

This Agreement applies to all volunteers associated with and/or involved in the activities or affairs of Vincare Services Foundation of Austin, Inc. dba Saint Louise House. This includes all activity associated with Saint Louise House (SLH) at its main office, on its properties and at all outreach site locations.

Any information concerning the identity of individuals receiving services of Saint Louise House and concerning the identification or address of any Saint Louise House (SLH) property is to be kept confidential at all times. The confidentiality agreement covers services rendered by all Saint Louise House Programs, any information about the individuals served by Saint Louise House and the information shared by participants in the course of receiving services. Client information, including all file information, is not be disclosed to any third party, under any circumstances, without the consent of the SLH employee that is supervising you and the Executive Director.

All pages, forms, information, designs, documents, printed matter, policies and procedures, conversations, messages (received or transmitted), resources, contacts, e-mail lists, e-mail messages, client, staff or public information is confidential and the sole property of SLH . This also includes, but is not limited to, any information of, or relating to, our staff, clients, operations and activities. This privilege extends to all forms and formats in which the information is maintained and stored, including, but not limited to hardcopy, photocopy, microform, automated and/or electronic form.

In order to protect the privacy and safety of the families at the Saint Louise House program, for purpose of this Agreement, "Confidential Information" shall include all information or photo images that would disclose the physical location or identifying structures of Saint Louise House.

Any disclosure, misuse, copying or transmitting of any material, data or information, whether intentional or unintentional, will subject you to disciplinary action and/or prosecution, according to the procedures set by SLH and any applicable laws.

By completing this application, I signify that I have read and understand this Agreement and that I agree to and will comply with these its terms.
