The Friends of the RUH and The Royal United Hospitals NHS Foundation Trust takes its responsibilities under the General Data Protection Regulations (2016) and Data Protection Act (2018) (subject to parliamentary approval) seriously; as such this form is used to record the explicit informed consent from you to use your personal identifiable information.

This means that knowing the risks, benefits and alternatives to providing and/or sharing your personal information, you decide to consent to your data being shared and provide relevant information. You have the right to remove your consent should you change your mind and can do this by contacting the Friends of the RUH Office Team located at B18, Royal United Hospital, Combe Park, Bath, BA1 3NG.

Personal information held by the Friends of the RUH includes: full name, postal address, email and telephone numbers. Additional information for volunteers includes : Start and End dates, Date of Birth, Emergency contact details, DBS number, National Insurance No, Training Completion Dates, Criminal Record Y/N, Occupational Health Clearance, Covid Vaccination status, details of volunteering activities.
