These are the requirements to volunteer at Holland Bloorview as a Family Leader:
- Be a:
- caregiver of a current/former Holland Bloorview client OR
- former client of Holland Bloorview (age 18+) OR
- caregiver of someone with a disability/developmental difference who participated in research done at the Bloorview Research Institute
- Commit to volunteering and logging on their volunteer profile, a minimum of 10 hours per year in the program
- Will complete and maintain all required qualification documentation and assigned training
- Participate in group information session and individual interview with the program staff
- Sign the Family Leader contract
I certify that the information in this application is correct to the best of my knowledge. I understand that any misrepresentation or omission may result in my dismissal if I am accepted as a volunteer.
