Should your application for voluntary help be successful, the Norfolk & Norwich University Hospital NHS Trust (NNUH) will need to hold and process personal information about you. This includes information, which is required for equal opportunities monitoring purposes. The data controller for this information is NNUH. This application and records management system is provided by Better Impact (https://www.betterimpact.com/privacy-policy/) as a data processor. For the purposes of a criminal record check the information you provide will be processed on TRAC; an application tracking system provided by Civica UK Ltd (https://www.civica.com/en-gb/policies-and-statements/privacy-notice/) as a data processor. Civica provide some recruitment support services and have staff and secure data storage located entirely in the UK.
I give my consent for the NNUH to hold and process personal information about me, in accordance with the above requirements.
I confirm that the above statements are true and correct, and understand that any misrepresentation will invalidate my application. All volunteers are required to have an Occupational Health screening. I am prepared to undergo a medical examination if requested.
CONFIDENTIALITY
I understand that any matters of a confidential nature, in particular information relating to the diagnosis and treatment of patients, individual staff records and detail of contract prices and terms, must under no circumstances be divulged or passed on to any person or persons. A breach of confidentiality may result in termination of the placement. I understand that there will be no payment for duties performed in a voluntary capacity and that it may be necessary to change my place of duty within the hospital.
