* As mandated by City of Burleson policy, we do not accept court-appointed community service for adults.
Situations arise within City departments that require employees and volunteers to deal with confidential information. Volunteers will be expected to maintain the utmost professionalism when dealing with information and records that are confidential.
Release of Personal Data: I hereby authorize any investigator or duly accredited representative of the City of Burleson to obtain any information from criminal justice agencies. This information may include, but is not limited to driving, arrest and conviction records. I hereby direct you to release such information upon request of the bearer. I understand that the information released is for official use by the City and may be disclosed to such third parties as necessary in the fulfillment of official responsibilities.
By completing this application, I certify that the provided information is true and correct to the best of my knowledge, and is being provided voluntarily for the purposes of applying as a volunteer for the City of Burleson CERT program.
I understand that acceptance into the program is contingent upon acceptable review of information, including but not limited to criminal conviction history, driving record, and any other such reports that may exhibit information about me. I acknowledge that any omissions, misstatements, or falsifications of my application will permanently disqualify me from participation with the City of Burleson CERT program.
I knowingly and voluntarily authorize and consent to allow the City of Burleson to request information from various Federal, State, and other agencies which maintain records concerning my driving history, criminal history, civil history, and other experiences about me. I hereby authorize without reservation any party or agency contacted by the City of Burleson, as a condition of volunteer appointment, to furnish the above listed information and to release and hold harmless all parties involved from any liability and responsibility for doing so. This authorization and consent shall be valid in its original, faxed, or copy form.
I knowingly and voluntarily authorize and consent to allow the City of Burleson to utilize my image or likeness for promotional purposes, both electronically and in print without compensation to me.
I acknowledge that my information may be used and disclosed for Emergency Management purposes; and that as a City of Burleson Volunteer, I am not an employee and will not be paid for my services. My participation in the program is “at will”, and may be revoked at any time by the CERT coordinator.
