Volunteer Requirements
JSSA volunteers serve adults and seniors of every religious background, race, and ethnicity. Please complete this application form if your child is interested in participating in a JSSA Heyman Interages Center program. By completing this form, you are granting your child permission to participate in a JSSA Interages program. Please note that only individuals 18 and older should fill out this form. Parents should fill out the form for children under the age of 18, and if you are a parent of multiple children who would like to participate, please fill out one form for each child.
Please note that youth volunteers are required the following prior to assignment:
- Must be a student enrolled in Grade 1 through 6
- 1 to 12 Month Commitment (varies by assignment, with most assignments being a minimum of 6 months)
- Agree to complete assigned activity
- Interview/Screening
- Additional Trainings
- Photo Release Acknowledgement
You agree that the JSSA® Heyman Interages Center may take photographs and or record videos of your child and use those photographs and videos in newspapers, newsletters, magazines, and brochures as well as on television, the JSSA Website, social media, and in other media, including those of other organizations. You understand that in most cases your child will not be identified by name in any video or photograph or notified of its use.
