Foster Agreement
This agreement is made and hereby entered into effect today by and between COLORADO PUPPY RESCUE (hereinafter referred to as “CPR”) and applicant (hereinafter referred to as “caretaker”). A foster home is defined in the PACFA rules and regulations as a site which, through a written agreement with a licensed rescue facility, provides care and/or rehabilitation for sick, injured, orphaned, juvenile or other pet animals which are otherwise the responsibility of the rescue facility. No more than 8 adult dogs or cats may be housed in any foster home at any time. A female dog with puppies under 8 weeks of age or a queen with kittens under 8 weeks of age will be considered as 1 adult.
Caretaker represents that he/she is at least 18 years of age and has reasonable knowledge regarding the proper care and handling of dogs, specifically puppies under 6 months of age.
Caretaker understands and agrees to the following terms and conditions:
- Caretaker has read the CPR Foster Volunteer Manual and understands the procedures and policies of the program.
- To temporarily house and care for any dog belonging to CPR. Caretaker understands that CPR is the sole and rightful owner of said foster dog.
- To accept and consider the dog as a household companion, not as an outside pet, and to provide him/her with humane care and treatment, including proper food, water, shelter and exercise, and to give him/her a reasonable amount of time to adjust to new surroundings.
- All decisions regarding said foster dog shall be made by CPR. CPR will consult with caretaker.
- To obey all applicable laws governing control and custody of animals
- Caretaker shall not give or otherwise transfer custody of foster dog to any other person, unless approved by CPR in advance.
- If for any reason caretaker is unable to continue to house and care for said dog, that he/she will notify CPR and will return said dog to CPR.
- CPR is not responsible for any damage caused by said foster dog to caretaker’s property or possessions.
- Caretaker understands and acknowledges that he/she is responsible for all expenses incurred as a result of fostering a puppy with the following exceptions:
- Colorado Puppy Rescue will provide an initial health examination and vaccinations and medications for existing medical conditions.
- Colorado Puppy Rescue will provide veterinary care with our vet tech and veterinarians.
- Colorado Puppy Rescue will provide food and a care package including crate, puppy toys, id collar, etc. when funds are available.
Any other expenses will be at the foster caretaker’s expense to include damage to the caretaker’s property or possessions.
- In the event that illness or injury should occur to the dog CPR will be notified immediately.
- Only CPR can authorize medical attention or medical bills to be paid.
- All animals belonging to caretaker personally must also be provided with all necessary medical care including vaccinations and parasite preventative. Parasite prevention should include intestinal and heartworms, fleas, ticks, and chewing lice.
- All animals belonging to caretaker personally which are not the property of CPR shall be spayed or neutered.
- Caretaker understands and agrees to adhere to all relevant zoning and animal control codes and ordinances, whether local, county, or state.
- CPR does not warrant the temperament or behavior of said foster dog and CPR is not to be held liable for any acts of said foster dog while in the custody of caretaker.
- In the event a foster dog becomes lost or is stolen, caretaker shall immediately notify CPR.
- Foster dogs must be supervised at all times when children and other animals are present.
- Foster dogs are never to be left outdoors while caretaker is away from home.
- No foster dogs will be allowed to roam at will off leash at any time.
- Caretaker must separate all animals in their possession when caretaker is away from home. Proper separation includes keeping each animal in its own crate and/or secluded in a secured area in caretaker’s home.
- Caretaker agrees to a home inspection within two days of entering into an agreement with CPR.
- CPR reserves the right to inspect caretaker’s home at any time.
- Caretaker acknowledges that his/her residence, if rented, permits pets and may be asked to produce a signed lease between caretaker and his/her landlord stating as such.
- Caretaker will provide a copy of his/her driver’s license in order to prove identity and residence.
- Caretaker shall notify CPR within 24 hours of any change in address, phone number or e-mail address.
- If caretaker requires an outside party to temporarily care for said foster dog, CPR must be notified in advance of temporary caretaker taking possession of said dog. If CPR feels that the temporary caretaker is not suitable, CPR shall take possession of said foster dog until such time as caretaker can resume full custody.
- CPR recognizes the right of caretaker, if so desired, to be included in the placement process. All CPR policies must be adhered to.
- Caretaker will not provide personal contact information to adopters for any reason. Adopters can be given the email address volunteer@coloradopuppy.org and the CPR facebook page web address for puppy updates.
- Caretaker may permanently adopt said foster dog if such placement is approved by CPR and provided that CPR is notified of the intent to adopt by noon on the day prior to the next adoption event.
- Any items borrowed from CPR such as crates, etc. must be returned to CPR when Caretaker ceases fostering each dog.
- Caretaker will keep appropriate medication and/or behavioral records on the foster dog while in your care and will present a profile of the dog to CPR upon return.
This contract represents the entire agreement between parties and any modifications will be made in writing and signed by both the foster caretaker and a representative of the foster home management team.
The Unfortunately Necessary Stuff...
I certify that I have read and understand the following:
- I understand that if I am injured while acting as an unpaid member of the volunteer staff, I am not covered by the Colorado State, Workers Compensation Law.
- I understand that every CPR volunteer must attend an orientation prior to volunteering at an event or fostering a puppy.
- I understand that if I have any physical, medical, or psychological limitations or disabilities that might hinder me from participation in any area of this program, it is my responsibility to inform CPR management.
- I understand that, due to the nature of CPR's work, I may need to communicate via mobile text message. If this is not an acceptable means of communication, it is my responsibility to inform CPR management.
- I understand that CPR is a PACFA licensee in GOOD standing and while puppies are in my care I must abide by PACFA regulations and Colorado Puppy Rescue guidelines at all times. Failure to do so may result in dismissal from the foster program.
- I certify that I am 18 years of age or older.
